The PATH Case Manager performs case management duties such as client intakes, treatment and discharge planning, individual, family, and group counseling, follow-up care, and crisis intervention.
Bachelor’s degree in Social Work, Psychology, Addictionology, or a health related field with at least two years of post-degree experience working in behavioral health. Past work in Assertive Case Management, partial care, residential substance abuse, emergency services and aftercare is preferred. May require nationally recognized substance abuse certification. Must possess a good driving record, valid driver’s license, and a registered four-wheeled vehicle.
